For armor transmogrification, approach Fergus Graem, the dwarf armorer located in Crow's Perch. Inquire about mastercrafted armor, and Yoana will then task you with a journey to Undvik in Skellige to retrieve essential blacksmithing tools. Upon successful completion of this endeavor, Yoana will become available to modify your armor's appearance.
Similarly, for sword transmogrification, visit Éibhear Hattori's shop in Novigrad, situated northwest of Glory Gate. Initiating his quest, "Of Swords and Dumplings," will allow you to unlock his capabilities as a swordsmith and, subsequently, his transmogrification services.
Once both Yoana and Hattori have been recruited and their respective quests fulfilled, simply navigate to the "reforge" tab within their crafting or purchasing interfaces. From there, you can select the desired visual style for your equipped armor or swords. Each cosmetic change will typically cost between 100 and 200 coin.
